AFFIDAVIT OF THE MASTER OR COMMANDING OFFICER, OR FIRST OR SECOND OFFICER

I, James W. Murphy, Master, of the S.S. "NEWFOUNDLAND", from Liverpool, England, do solemnly, sincerely, and truly swear that I have caused the surgeon of said vessel sailing therewith, or the surgeon employed by the owners thereof, to make a physical and mental examination of each and all aliens named in the foregoing Lists or Manifest Sheets ____ in number, and that from the report of said surgeon and from my own investigation, I believe that no one of said aliens is of any of the classes excluded from admission into the United States by laws regulating immigration, and that also, according to the best of my knowledge and belief, the information in said Lists or Manifests concerning each of said aliens names therein is correct and true in every respect.

I, E. Austin Gibb, Surgeon of the S.S. "NEWFOUNDLAND", Sailing herewith do solemnly, sincerely and truly swear that I have had 23 years experience as a Physician and Surgeon and that I am entitled to practice as such by and under the authority of the General Medical Council and that I have made a personal examination of the aliens named herein and that the foregoing Lists or Manifest Sheets, ____ in number, according to the best of my knowledge and belief, are full, correct, and true in all particulars, relative to the mental and physical condition of such aliens.

14. Final destination, State, City or Town;
15. Whether having a ticket to final destination;
16. By whom was passage paid,
17. Whether in possession of $50 and if less, how much?;
18. Whether ever before in the United States, and if so when and where,
19. Whether going to join a relative or friend, and his name and complete address;
20. Whether alien intends to return to country whence he came after entering temporarily in laboring pursuits in the United States;
21. Length of time alien will reside in US;
22. Whether intends to become a citizen;
23. Ever in prison or in an almshouse;
24. Whether a polygamist,
25. Whether an anarchist;
26. Belief in overthrowing the Government of US;
27. Promise of job in US;
28. Previously deported from US;
29. Condition of health, mental and physical;
30. Disabled or crippled, Nature Length of time and cause;
31. Height;
32. Complexion;
33. Colot of Hair;
34. Color of Eyes Eyes;
35. Marks of Identification.
