Affidavit of the Master or Commanding Officer or First or Second Officer I II Officer of the Cincinnati from Genova do solemnly, sincerely, and truly swear that I have caused the surgeon of said vessel sailing therewith or the surgeon employed by the owners thereof to make a physical and oral examination of each and all of the aliens named in the forgoing Lists or Manifest Sheets 29 in number, and that from the report of said surgeon and from my own investigation, I believe that no one of the said aliens is an idiot, or imbecile, or a feeble minded person, or insane person, or a pauper, or is likely to become a public charge, or is afflicted with tuberculosis, or with a loathsome or dangerous contagious disease, or is a person who has been convicted of or who admits having committed a felony or other crime or misdemeanor involving moral turpitude or is a polygamist or one admitting belief in the practice of polygamy or an anarchist, or under promise or agreement express or implied to perform labor in the United States, or a prostitute, or a woman or girl coming to the united states for the purpose of prostitution, or for any other immoral purpose, and that also, according to the best of my knowledge and belief, the information in the said List or Manifests concerning each of said aliens named therein is correct and true in every respect.

Zeaulieu, Surgeon of the SS Cincinnati sailing therewith, to solemnly, sincerely and truly swear that I have had 12 years' experience as a Physician and Surgeon, and that I am entitled to practice as such by and under the authority of the Yenusen Geouvineuh and that I have made a personal examination of each of the aliens named hereto, and that the forgoing Lists or Manifest Sheets 2 in number, are, according to the best of my knowledge and belief, full, correct, and true in all particulars relative to the mental and physical condition of such aliens.
Sworn to before me this 17th day of April, 1913 at New York.

Transcriber Notes: * is for error by the original recorder(s) or to note other information on the copy, added to the transcriber's notes. ? Transcriber unable to read due to the quality of the copy, letters are too small, written over or elaborate writing. This Manifest indicates the ship left Genova on 2 April, Naples on 3 April, and Palermo on 4 April. All passengers have checks before surname between age and gender columns, and by Final Destination, except for those crossed out passengers 115, 123, and 136 , with no check. This usually means that a passenger was not on this ship. First passenger is crossed out, unable to read the Nationality or Race, both are written as do or ditto for all on the page, but most likely all are Italian and Italy. Second passenger, Race is South, all after written as do. Recorders used the word do, or ditto, to repeat information from a line above for related passengers in columns from Able to Read and Write to Final Destination. Transcriber has replaced the dittos with said information on the list. On the bottom of this page, from passengers 140 to 144, over the columns of last residence, has numbers 27 over a line, and 26 under it, like a fraction. May refer to the number of passengers on this page finally making the voyage. On top of the page, in given name column title the number 15 is stamped, at the bottom of the same column a 12 is stamped. The Italian language has no letter j, k, or w. The letter h is only used with ch or gh. The passenger listing above is only one page of this manifest. Both pages can be seen on the Ellis Island Web Site. Surname looks to begin with Ag, end in it, maybe Agostini, as nearest relative or friends, but there is no relationship given. Given name may begin with an S, rest looks like wavy lines, unreadable. Nationality most likely Italian, as Last Residence is Italy, Milan. Race, unable to read, maybe North, as next line is written as if changed to South. Nearest Relative name is written above line. Underneath line is either no 5, 3 or no S Gregorio 25, as current Italian addresses have the number after the street. In Transit to, is written above cross out line, below is written, Callas. 115 to 116 Between these passenger lines, from occupation to Nationality columns numbers are written, 23 dash 85312 a dash and space, then 7 dash 31 dash 40. 119 Antoni, there is an x marked in the given name column, just before the age column. Marital status is written over, maybe single or married. Nearest Relative, looks like Odsardo, no name like it found. 120 Barsotti, columns from Occupation to nearest relative have words written over the data there, larger than origional script, and columns from Nationality to residence have a thick line over the original words. Looks like Cit then letters transcriber cannot make out, then Contry Comt or Conit, and Buffalo, NY. Also in nearest relative column are numbers first batch transcriber cannot read, nest group look like 11 dash 1903, or 11 dash 1903, or 117903. Nearest relative address looks like Bagni or Bagini. Final destination is written as do or ditto. 121 Barbieri, Last Residence town may be Carrara or Cazzara. 122 Bianchi, Nearest relative column address. La Ru?a, letters are small and unclear, maybe La Ruga or La Rosa, as a letter from passenger below touches the bottom of this letter. Destination, Yowa Belle Paine is most likely Iowa. 123 B?, ? This passenger is crossed out by a thick line, usually meaning they did not board this ship. Transcriber can read only what is stated above. Surname & given name have an extra wavy line drawn through, unable to read. Age 2? last digit is blocked by line maybe 23, 25, or 28. Occupation is written below the line, as is able to read and write. Rest of columns are written as do or ditto above or below the line out. Surname of nearest relative is clearer and above the line, maybe Boreini or Bonini. Final destination is written below the line, which is lighter. 124 Brocchini, typed before surname is ADMITTED, and letters S and t, or S and a check mark are over the beginning of the word. Nearest relative seems to be maiden and married name, preceded by word in, may mean in, into, at, or upon. 127 to 130 Dellafelice, is written as one word for passengers and Della Falice for nearest relative, may be recorders mistake. Before the age columns there is a vertical curved bracket pointing back to given names with writing before it. Passenger 128 has wife, 129 has Bro for Brother, than 130 has wife no. 15, as the line on the manuscript. Columns from Nationality to destination are written as do or ditto. Nearest Relative address, Se??i?gl????i, letters are small and close together looks like it maybe begin Serrim or Sezznn, rest is unable to read. May be a small section of the town Pasaro. 130 to 131 below Dellafelice and within Dell' Orso space in family and given names column is written AC or PTC Naples, then numbers written as a date would be, a 3 slash 22 slash 37, then a circle around an m, then a dot, a P, dash 1183194, then as if date 3 slash 2 slash 38. In the Occupation to Nationality columns are more numbers, 22 dash 67391, a space and again as if a date, 11 slash 27 slash 37. 131 Dell 'Orso, Destination city is written over in another hand, changed from S Francisco. Second word looks like it begins with a European style 7, with a horizontal line through it, or it maybe an F. Second word is light a bit longer and has 2 taller letters, looks like F ?nlalin??, but either of these letters maybe an b, d, h, k, or a t, unable to tell. 132 E??i, writing small, looks like Equi, but maybe Eqni or Egni. Nearest relative looks more like Egni. 133 to 137, Fontana, All surnames 134 to 137 are written as do, as are columns from Race to destination. Before the age columns there is a vertical curved bracket pointing back to the names, can make out only a few pairs of letters, one below the other. First, third and fourth look like Ch, short for child, second looks like ule, but maybe sloppy Ch. Teresa and ?uiniti?ia, occupation is written as do, from Luigi above. 133 Fontana, within can write column, but after yes, are a right then left arrow, like on the comma and period of a computer keyboard, looking like a broken square. 136 Fontana, ?uiniti?ia, looks like Quintilia, but could be Guintilia, no names found beginning with Q, nor any similar found. This may be a nickname or a particular regional name. 137 Fontana, under occupation is written none, and in able to read and write column are two dashes, instead of no. 138 F????????, passenger columns are crossed out, usually meaning they did not board this ship. Line covers most columns, but given name is readable, as are some letters of surname, first four are Fran, last two look like hi. Name for number 139, was written as do or ditto then crossed out, surname may also be Franceschi. Age looks like it maybe 25 or 35, Occupation maybe laborour. Name of nearest relative surnames look like Squaglia Francesehi or Franceschi, first is most likely her maiden name. 139 Franceschi, the word do or ditto written then crossed out for surname, as 139 was crossed out. Occupation written as do, and line above maybe labouror. Wife Zai?a, maybe Ziara, Ziaza, or Ziasa. Final destination, looks like first writen a do or ditto. Both are written over, looks like Cureka, Cuzelsa, but could be Eureka. 140 Francioni, U?ile, looks like Uneile, but could be Umile, no name similar found, except Emile but first letter is a U. 141 Francioni, Given name maybe Leonida, or Leoinda, no name like it found. A bracket is before ages of 139 and 140, with Ch written before it, most likely for child. Nearest relative, only father is written. All columns from race to final destination, written as do or ditto from 139. 143 Gonnella, Given name, has an ink blot over z of Bonifazio, maybe Bonifario, no name similar found. Nearest Relative, Gioco?da, no name similar found, looks like Gioconda, but maybe Giocorida. Destination, Yowa, Councul Blaffco, most likely Iowa, city of Council Bluffs. 144 Lencioni, Occupation is either Cooper, Cosper or Cosfer. Nearest relative residence maybe Toringo, or Tozingo. Final Destination is written over, looks changed from S Francisco to thicker but lighter writing. First letter looks like a European style crossed 7, but may be an F then an N. Can not read the rest, writing seems to fade out, but looks to end in nge, ngh or augh.
