I, J. S. Theobald do solemnly, sincerely and truly swear that the Report or Manifest subscribed with my name, and now delievered by me to the Collector of the District of Philad. contains, to the best of my knowledge and belief, a just and true account of all the passengers which were on board the Ship Thomas H Perkins at the time of her sailing from the port of Liverpool or which have been taken on board at any time since; that I am at present, and have been during the voyage, master of the said vessel. And I do likewise swear that all matters whatsoever in the said Report or Manifest are, to the best of my knowledge and belief, just and true. (signature) Jas ? Theobald

Transcriber's Notes:

* The use of an asterisk * indicates a note in the Transcriber's or Passenger's Notes.
* The use of a ? indicates a word or letters that could not be read, due to quality of 
  original document.
* Passengers 18-32 have no occupation listed.  There was nothing indicating (ie. ditto 
  marks) they were laborers as was the first person on the page, but most likely they 
  were.
* Passengers 34-46 have no destination listed.  The first person on the page had United
  States but there were no indications (ie. ditto marks) for these passengers.
* Passengers 45-46 did not have sex listed.


 1  This passenger is clearly listed as 2-1/2 years of age, and also clearly labeled a 
    Servant.
27  The age column was left blank.
31  The surname is either Rud or Reed.
40  Surname could be Hansse.
42  The first name appears to read 'Maryrude' or 'Marynde'; the letters are relatively
    clear, but this is an unknown name.  It is not Maynard.
43  Her age was originally 2, but over written with 24.
44  The age was originally 24, but was over written with '1'.
