I, Wm. Prothero Chief Officer of the Carpathia from Liverpool, do solemnly, sincerely and truly swear that I have caused the surgeon of said vessel sailing therewith or the surgeon employed by the owners thereof, to make a physical and mental examination of each and all of the aliens named in the foregoing Lists or Manifest Sheets, 8 in number, and that from the report of said surgeon and from my own investigation I believe that no one of said aliens is an idiot, or imbecile, or a feeble minded person, or insane person or a pauper, or is likely to become a public charge, or is affected with tuberculosis, or with a loathsome or dangerous contagious disease, or is a person who has been convicted of, or who admits having committed a felony or other crime or misdemeaner involving moral turpitude, or is a polygamist, or one admitting belief in the practice of polygamy, or an anarchist, or under promise or agreement, express or implied to perform labor in the United States as a prostitute, or a woman or girl coming to the United States for the purpose of prostitution or for any other immoral purpose, and that also, according to the best of my knowledge and belief, the information in said Lists or Manifests concerning each of said aliens named herein is correct and true in every respect. Should be Vt. 233 Charles Short is also entered after passenger #113 but the entire line is crossed out. 234 Family name Yoder is written thus: "Yo\der", with the third letter crossed out with a slash. 235 Last word in Going to column is too faint to read. 236 Mother's last name could be Aselain or Nielsen or something close. Scrawled writing is difficult to read. 240, 241 Final destination city reads "Fi" followed by letters that could be "nnie" or "nice" or "nme" or "nsmc" or some combination of these letters. 242, 243 Identified as US Citizens. The destination for these two passengers is one entry bracketing both. The street name is very faint and cannot be determined. 249 First name of nearest relative might be "Surgeon". Also, there is a check mark in the Marks of identification column, used as a ditto from previous line, but it is not clear that the original clerk meant that this passenger also had a birthmark on his face, since the birthmark comment in the line above may have been added after the original clerk's entry.
