DISTRICT OF BALTIMORE - PORT OF BALTIMORE
I, J G Smith do solemnly, sincerely, and truly swear, that the within list, subscribed with my name, contains to the best of my knowledge and belief, a just and true account or report, of all the passengers who have been taken on board the Ship Matilda at London or any other foreign port, or at sea, and brought in said Ship Matilda into any District of the United States, since the departure from said port of London.
