SS City of Havana
I, S. T. Phillips, Master of the above-named City of Havana, from the Port of Vera Cruz, do solemnly, sincerely, and truly swear that the above List contains the names and description of all the Passengers who were on board the said S.S. City of Havana at the time of or since her last departure from the said Port of Vera Cruz, or that have been taken on board the said vessel at any foreign port or place, and that none have died on the voyage.
