I, Dr. Ludeke, Surgeon of the "Kaiser Wilhelm der Grosse" sailing therewith do solemnly, sincerely, and truly swear that I have had fifteen years experience as a Physician and Surgeon, and that I am entitled to practice as such by and under the authority of the university of Munster, and that I have made a personal examination of each of the aliens named herein, and that the foregoing Lists or Manifest Sheets, 25 in number, are, according to the best of my knowledge and belief, full, correct, and true in all particulars, relative to the mental and physical condition of such aliens.

Required by the regulations of the Secretary of Commerce and Labor of the United States, under Act of Congress approved February 20, 1907, to be delivered to the United States Immigration Officer by the Commanding Officer of any vessel having such passengers on board upon arrival at a port in the United States.

Correspondence 12 April 2004: In April, 1906, Antonin (Tony) DORNAK left his home in Valasska Senice, near Francova Lhota, Moravia (which was then part of Austria but is now in Zlinsky Kraj, Czech Republic). His first voyage was on the SS Main from Bremen to Ellis Island. First settling in Cudahy, Wisconsin, he later moved to Minneapolis, Minnesota. About four years later he returned to Valasska Senice to marry his childhood sweetheart, and next door neighbor, Filomena ZIMEK. They returned together on the SS Kaiser Wilhelm der Grosse, living the rest of their lives in Minneapolis. Less than six months after their arrival, Filomena gave birth to premature twin daughters who died shortly after birth. Other children born to Tony and Filomena: Alice, Amelia, Anthony, Mary and two sons who are still living. Other DORNAKs were in Cudahy, Wisconsin in 1910. Also, Filomena's brother, Frank ZIMEK, came to the US, first to Minneapolis, then Texas, eventually settling in Indiana. Surnames (accents not included here) of ancestors of Antonin and Filomena, all from Francova Lhota and the surrounding areas of the Czech Republic near the border of Slovakia (including Valasska Senice, Lidecko, Strelna, Valasske Klobouky and Vsetin) are: Dornak/Dornakova, Janac/Janacova, Zimek/Zimkova, Psota/Psotova, Kurtin/Kurtinova, Liska/Liskova, Janigac, Pracharova, Repa/Repova, Martink/Martinkova, Frantis/Frantisova, Hrib/Hribova, Vrana/Vranova, Juran/Juranova, Pikalek/Pikalkova.
