Transcriber's Notes:

The use of an * indicates an omission or error made in the original document
or a clarification by this transcriber.

The use of a caret "^" indicates the following letter has been
superscripted.

The signature attesting to the sworn statement is indecipherable; it is
followed by "DC" which indicates the Deputy Collector of Customs

*The captain's name as written on left of document contains a "leading s";
 so it looks like "Bafsett". Note that when twice signed by the captain, the
 letters are definitely "ss". I believe his first initial is a "F"; however
 it may be an "T".
