Liverpool, England to Baltimore, Maryland

DISTRICT OF BALTIMORE - PORT OF BALTIMORE
I, J. Fuller, Jr., Master of the Ship Harvest of Liverpool, do solemnly swear that the annexed List contains to the best of my knowledge and belief a just and true account or report of all the passengers taken on board the said ship at Liverpool or any other foreign port, or at ?????? in the said vessel into any District of the ?????? departure from the said Port of ??????.
