COMMISSIONER OF IMMIGRATION
Required by the regulations of the Secretary of the Treasury of the United States, under Act of Congress, approved March 3, 1893, to be delivered to the Commissioner of Immigration by the Commanding Officer of any vessel having such passengers on board upon arrival at a port in the United States.

If yes, state which; 18) Whether a Polygamist?; 19) Whether under Contract, Express or Implied to Labor in the United States?; 20) Condition of Health, Mental and Physical; 21) Deformed or Crippled, Nature and Cause.

  February and March are abbreviated in "Statement" as "Feby and Mch".  
  Passenger 1, nothing noted in column 12.  In column 14 is written
    "over 30" with "$150" written over it.
  Passenger 3, in column 14 is written "16.00" with "$13.00" written 
    over it.
  Passenger 6, middlename is unreadable.
  Passenger 17, "given name" is unreadable.  
  All Passengers have double entries in column 15.
  No passengers died on this voyage.
