AFFIDAVIT OF THE MASTER OR COMMANDING OFFICER, OR FIRST OR SECOND OFFICER
I, E. Bent, Master of this Jap. SS Hong Kong Maru from Honolulu, do solemnly, sincerely, and truly swear that all the aliens embarking at an insular port are listed on the within manifests, and that, according to the best of my knowledge and belief, the information in said Lists or Manifests concerning each of said aliens named therein is correct and true in every respect(S) E. Bent, Master Sworn to before me this _____ day of _______190 at San Francisco. ____________________________ Immigration Officer
List or Manifest, of Alien Passengers from U. S. Insular Possessions, for the United States Immigration Officer at Continental Port of Arrival.
Required by the regulations of the Secretary of Commerce and Labor of the United States, under Act of Congress approved February 20, 1907, to be delivered to the United States Immigration Officer by the Commanding Officer of any vessel having such passengers on board upon arrival at a continental port in the United States.

First letter of family name is difficult to determine; however, its position between S&W in an alphabetized list gives credence to its being a "T". The fourth letter could be a "p". * #5&6(second).Toney Forde & Teddie McNamara - A checkmark in the sex column obliterated the entry; likely they are boys, according to position in list.
