Transcriber's Notes: 
* captain's name could be Luck or Lieck.
* #2-5 last 2 columns had wavy lines drawn in them.  Probably meant to 
  indicate ditto.
* #6-9 nothing in last 2 columns.
* #9 age either 3 or 5.
* #11-12 very long word for occupation.  First few letters could be musi.
* #13 age not given.
