DISTRICT OF NEW YORK - PORT OF NEW YORK
I, J Brown Master of the SS Cornwall do solemnly, sincerely, and truly Swear that the following List or Manifest, subscribed by me and now delivered by me to the Collector of the Collection District of New York, is a full and perfect List of all the Passengers taken on board of the said SS Cornwall at Avonmouth from which Port said Steamer has now arrived, and that on said List is truly designated the age, the sex, and the occupation of each of said passengers, the part of the Vessel occupied by each during the passage, the country to which each belong, and also the country of which it is intended by each to become an inhabitant; and that said List or Manifest truly sets forth the number of said passengers who have died on said voyage, and the names and ages of those who died. List of Manifest Of All The Passengers taken on board by SS Cornwall whereof J Brown is Master from Avonmouth burthen 1841 tons.
Columns represent: Names, Age, Sex, Occupation, Country from Country intended to become, died on voyage part of the vessel occupied by each passenger during the voyage. All Passengers were from England and going to US America

Transcriber's notes: 

* In the death column the Captain has written "nil" once  and ditto marks for the 
  remainer of the list.
* The Agents name is very crudely written.  After the R could be an a or e the next 
  letter goes below the line.  the next two letters could be nn  next letter ? another below and above line and a,e,o, 
* #4 It is abreviated as Domes. Servant
