Transcriber's Notes:
It is not possible to read the two initials at the beginning of the
  Collector's signature.
The words "United States" are written at the top of the "Country in
which they intend to become inhabitants", with ditto marks underneath
for every passenger on the voyage. I did not include that column in
  my transcription.
The word None is written in the center of the "Died on the Voyage"
column, indicating that no passengers on this voyage died. This is
why I omitted the column in my transcription.
The surname for Passenger #5 could be Gunsen or Gunson. The copy is
  too light to be legible.
The surname for Passenger #12 could begin with either an H or a Gl.
The last letter could be an n or a w.
The entire name for Passenger #16 is illegible. The paper had
apparently been folded just at his name, and it seemed to have cracked.
The copy just shows dark spaces over most of the letters. It is
possible that the given name is John, but it is impossible to tell
definitively. The occupation may be Mason. The crack does not
obliterate that word, but the copy is too light to be legible.
